Pre-Kindergarten Screening 

Process: Pre-Kindergarten screening is intended for families wishing to enroll children in the four-year-old class at Palmer Center or in one of the four-year-old classes housed in city elementary schools. These programs are open only to city residents. 

Note:  Children who are five (5) years old on or before August 31, 2013 will need to enroll in Kindergarten at the school for which they are zoned.

• Childs' CERTIFIED Birth Certificate (A "mother's copy" is not acceptable.) If the parent does not have a certified copy, contact the Health Department, Vital Records at 1-423-279-2780.
• Social Security Card (for the child) If the child does not have one, the parent should contact the Social Security Administration at 1-800-772-1213.
• Proof of Income (Since our Pre-k programs are supported with state and federal grants, we must enroll low-income students first. Families that can verify low-income status are given preference in the enrollment process. Therefore, we need to verify total family income. The documents listed below are acceptable for that purpose.)
  • Income Tax Form 1040A or 1040 (last year)
  • W-2 Form (from last year)
  • Letters from the Department of Human Services Verifying Income
  • Food Stamp Status Number (10 digit number)
  • Temporary Assistance to Needy Families (TANF, Families First, Transitional Families First (case number)
  • Pay stubs/records (statements of earnings and deductions from the last 30 days)
  • Child Support status verified through the Child Support Enforcement agency in Nashville may be used as part of the income verification process. However, documentation of other family income from sources such as a W-2 or Income Tax forms must accompany the child-support information.
  • Unemployment Compensation (Verification Letter) Other family income must alsobe verified.
  • Supplemental Security Income (SSI) verification
  • Employer letter verifying pay (The letter should include)
    • How long employed
    • Hourly Rate
    • Supervisor's signature
    • On Company/Business Letterhead
• Proof of Residency (Cell Phone bills are not acceptable.) Acceptable documentation includes any of the following:
  • Utility Bill (electric or water)
  • Rent Receipt
  • Mortgage Statement
  • Property Tax Receipt
• TN Department of Health TN Pre-School Immunization/Physical Certificate (yellow form) or TN School Immunization Certificate (green form) completed and signed by your child's doctor.
